  • Ovarian agenesis
  • Primary ovarian failure (Swyer syndrome, Turner syndrome)
  • Age-related declined ovarian reserve
  • Premature ovarian failure (secondary)
  • Radiotherapy or chemotherapy
  • Surgical removal of ovary
  • Multiple IVF failures
  • Chromosomal abnormality in karyotype of the woman
  • History of initially chromosomally abnormal children
  • Unaccessible ovaries for ovum pick-up, severe adhesion pelvic disease
  • Recurrent pregnancy loss

Sperm Donation
Indications of Donor Sperm
  • Non-obstructive azoospermia
  • Recurrent IVF failures due to poor sperm quality
  • High risk of genetic abnormality transmission
  • Infectious disease in male partner
  • Single mother desirous of child
  • Homosexual couple desirous of child
  • Severe Rh isoimmunization
  • Severe oligo-asthenospermia not suitable for ICSI
Requirement for a Sperm Donor
  • Must be free from HIV, hepatitis B & C, hypertension, diabetes, sexually transmitted diseases, and common genetic disorders such as thalassemia
  • Age of donor must be between 21 and 45 years
  • Semen analysis must be normal as per WHO guidelines for ART
  • Blood group and Rh status must be recorded
  • Information such as height, weight, age, education, profession, skin and eye color, medical history (including psychiatric disorders), and family history of genetic disorders must be documented in an appropriate proforma
